This program allows new or existing AHS Community Home Care clients to choose an eligible private home care provider like Care At Home Services for personal care services up to their AHS-authorized monthly hours. How to Get Started
- Check Your Eligibility: Contact your AHS Case Manager through 811 for an assessment.
- Sign the Necessary Forms: Complete the Consent to Disclose Information and the Letter of Agreement.
