Managing Your Hospital Stay

A hospital stay can feel overwhelming, from preparing for admission to making sure things at home are taken care of while you are away. Our care aides can support you before, during, and after your hospital stay, including help with packing, arranging transportation, and light home support. When you return home, we can also assist with daily routines and recovery support so you can focus on healing and feel more comfortable throughout the process.

Foot & Wound Care

Care At Home Services provides in-home skin and wound care support for people recovering after surgery and discharge, or for those living with chronic conditions such as diabetes who may be at risk of foot or leg ulcers. Our wound care nurses can assess needs, provide treatment recommendations, and help you and your family with ongoing wound care management, including support with:
Education and coaching to support self-care, including how to change dressings, recognize signs of infection, and maintain healthy routines such as nutrition and appropriate activity.
Wound monitoring through regular assessment and measurement, with recommendations on appropriate dressings and how often to change them, based on current clinical standards.

Care At Home Services provides assistance with patients managing home antibiotics and other antimicrobials as ordered by the patient’s physician. We work in conjunction with the community home IV programs and infectious disease teams, who oversee patient infusions to provide the safest and most convenient delivery system based on patient factors and drug stability. We supervise and monitor our patients for signs of complications and infections related to IV therapy. If you require home IV therapy, we can help make it safe and easy for you.

We develop specific home rehabilitation programs to meet the needs of each individual patient.

Physical therapy uses physical activity, exercise, and additional treatment, such as massage, to aid in strengthening, healing, and relieving pain after an injury, illness, or operation.

Occupational therapy is a rehabilitative service for those suffering mental, physical, developmental, or emotional conditions. It restores or maintains patients’ ability to perform every-day tasks, often through developing ways to modify or adapt activities.
