Arthritis Care

Arthritis can make everyday tasks and household routines harder over time, even with treatment. If you or a loved one could use support at home, Care At Home Services can help with both personal care and day-to-day assistance. Our clinical specialists can create an individualized exercise plan that may include low-impact aerobic activity, gentle strengthening, and range-of-motion exercises. If surgery is part of your care plan, we can also support recovery at home after discharge, including coordination of physiotherapy and occupational therapy.

Cancer Care

Care At Home Services provides compassionate cancer care and support for clients and families during active treatment. With your consent, we can coordinate with your oncology team, including BC Cancer and Alberta Cancer and local health authority staff, to support your individual needs and help with symptom management related to treatment. Support is tailored to your situation and care requirements, with 24/7 options available when needed.

COPD Care

Care At Home Services has extensive expertise in helping patients and their families live with Chronic Obstructive Pulmonary Disease (COPD). In addition to transitioning home after a hospital stay, we can also work with you, your physician, and family to customize a program that:

Provides medication reminders, assistance, administration and reconciliation, as well as oxygen therapy at home if necessary.
Teaches an individual how to recognize early changes in how they feel, and to take appropriate action to decrease episodes of serious illness.
Assists with physician follow up including transportation.

Diabetic Care

Care At Home Services knows that managing diabetes can be a full time job. That is why our care team provides diabetes care and management to patients and families who require assistance with:

Education and teaching to support self-care management of Diabetes Type 1 or 2.
Monitoring of capillary glucose and the administration of insulin and oral hypoglycemic medications if the patient is unable to do so or if the family requires respite support.
Clinical support for complications associated with the condition like diabetic neuropathy and associated wounds.

Congestive Heart Disease

Care At Home Services has experience helping individuals and their families live with advanced heart disease. We can customize a program that:

Provides medication reminders, assistance, administration and reconciliation.
Teaches individuals how to recognize early changes in how they feel, and to take appropriate action to decrease episodes of serious illness.
Develops and/or prepares a heart healthy diet (including shopping and meal preparation, if necessary).
Assists with physician follow up including transportation.
Helps with the daily activities of living such as bathing and dressing.

Multiple Sclerosis (MS)

Care At Home Services is experienced at helping individuals and their families live with MS. Our trained professionals can customize a program that:

Provides medication reminders, assistance and administration.
Helps with the activities of daily living such as bathing and dressing.
Provides relief and respite care to the patient’s caregivers by assisting with routine home management issues such as housekeeping or laundry.

Enhances independence with activities of daily living through occupational therapy.

Parkinson’s Care

Dementia & Alzheimer’s Disease Care

Alzheimer’s Disease affects every client and family differently, resulting in the need for a highly individualized approach to caregiving. Care At Home Services’ highly skilled staff will provide immediate and effective nursing care that is designed to meet the patient’s specific needs.

Our dementia care program is tailored to meet the unique needs, interests, habits and desires of our patients. We have adapted and endorsed the Alzheimer Society of Canada Guidelines for care: Person-centred care of people with dementia living in care homes framework. The tenants of the program include the following:

Promote respectful relationships and interactions with people with dementia.
Develop strong bonds with family members of people with dementia and engage them in activities.
Anticipate the needs and reactions of people with dementia and adjust individual, social and environmental factors to reduce negative behaviours.
Encourage and support persons with dementia to make choices in keeping with the person’s lifelong values, preferences, and interests.
